The cigarette may be disappearing from the lives of the young. But the kidneys may not be getting a break just yet. For generations, smoking and alcohol were familiar villains in health warnings. Cigarettes were linked to lung and heart disease; excessive alcohol to liver damage. The kidneys rarely received the same attention, even though they filter waste from the blood, regulate fluids and electrolytes and help control blood pressure around the clock. Now the lifestyle map is changing. Gen Z is often seen as more health-conscious than previous generations. Younger adults in several countries are smoking less, while some are also drinking less or choosing to delay alcohol. Fitness trackers, nutrition apps and social-media wellness content have made health information almost impossible to escape. Could that give Gen Z healthier kidneys as it ages? Possibly — but only if avoiding the old risks is accompanied by healthier everyday habits. Because while the cigarette may be disappearing, a different set of kidney risks may be quietly taking its place.
The kidney damage behind a cigarette
Smoking is usually thought of as a lung problem. The kidneys, however, are highly dependent on healthy blood vessels. Tobacco exposure can contribute to vascular damage, oxidative stress, inflammation and high blood pressure. It is also associated with protein appearing in the urine — proteinuria — an important marker of kidney injury. The concern becomes greater when smoking is combined with diabetes or hypertension, two major drivers of chronic kidney disease. A nephrologist explains that smoking can worsen vascular injury and blood-pressure control and may accelerate kidney damage, particularly in people who already have diabetes, hypertension or established kidney disease. And there is a reason kidney damage can remain unnoticed for years: the kidneys are remarkably quiet when they are struggling. A person can feel perfectly healthy while kidney function is gradually declining.
Alcohol: it is the pattern that matters
Alcohol presents a more complicated picture. The concern is particularly heavy or binge drinking. Large quantities can contribute to dehydration and disturbances in fluid and electrolyte balance. Add vomiting, poor food intake or prolonged dehydration, and the kidneys can come under considerable stress. In vulnerable people, acute kidney injury can follow. Repeated heavy consumption can also contribute to hypertension, liver disease and metabolic problems, indirectly increasing the burden on the kidneys. So the useful question is not simply: Do you drink? It is: How much, how often and in what pattern? That distinction matters for Gen Z too. Drinking less overall does not necessarily mean harmful drinking has disappeared from every social setting.
The Gen Z advantage — with a catch
If younger adults continue to avoid cigarettes and harmful drinking patterns, they could reduce their lifetime exposure to two established health risks. But nephrologists caution against declaring Gen Z the healthiest generation. A young person may never smoke, rarely drink and still spend most of the day sitting at a laptop. They may order most meals online, sleep at 1am, rely on energy drinks to stay awake and develop excess weight or high blood pressure without realising it. The risk profile may be changing rather than disappearing. And perhaps the clearest symbol of that change is not a cigarette packet. It is a chair.
The chair problem
For many young urban Indians, the day can move seamlessly from laptop to phone: office or college through the day, food delivered to the desk, gaming or streaming at night, and social media in between. Physical inactivity does not directly damage the kidneys. Its effects are more indirect — but potentially significant. Over time, inactivity can contribute to obesity, insulin resistance, hypertension and diabetes. These conditions can place sustained stress on kidney function. Regular physical activity therefore protects kidney health not simply by improving fitness, but by helping control the conditions that damage the kidneys. For a generation that can track its steps to the nearest hundred, the challenge may be less about knowing what to do and more about getting up and doing it.
When convenience becomes the diet
Then there is food. Instant meals, packaged snacks, processed meats, fast food and sugary drinks have become easy options for time-pressed young adults. An occasional takeaway is unlikely to determine kidney health. The concern is what happens when convenience becomes routine. A high-sodium diet can contribute to hypertension. Excess calories can contribute to obesity and metabolic disease. Both can increase long-term kidney risk. And kidney health is not simply about drinking more water. Hydration matters, but so do salt intake, blood pressure, blood sugar, body weight and the overall quality of the diet, nephrologists point out. For someone meticulously counting protein while ignoring sodium, the health equation may be incomplete.
The fitness paradox
Gen Z's growing interest in fitness brings another question: protein powders, pre-workout products, energy drinks and supplements. Protein is not automatically harmful to healthy kidneys when consumed appropriately. But very high-protein diets may not be suitable for people with underlying kidney disease. The concern is often unsupervised use — assuming that something marketed for fitness must be safe simply because it is popular. Nephrologists advise people with kidney disease or other medical conditions to seek individual guidance before adopting very high-protein diets or multiple supplements. Energy drinks bring another issue. Excessive caffeine can interfere with sleep and may affect blood pressure in some people. Which brings us to another modern kidney-health concern: sleep.
The sleep debt
Late-night work, gaming, streaming and scrolling can make sleeping at midnight seem normal. Poor sleep is not a direct kidney toxin. But chronically inadequate sleep can influence weight, metabolic health and blood-pressure control — all factors that eventually matter to the kidneys. The kidney, in other words, may not be the first organ affected by an unhealthy lifestyle. It may simply be one of the organs that pays the price later.
Three generations, different risks
Millennials sit between the two worlds. They witnessed the arrival of smartphones, food-delivery platforms, desk-based work and an always-connected lifestyle. Many are now entering their 30s and 40s, when hypertension, obesity and diabetes can become increasingly relevant. Boomers may carry a different legacy: decades of exposure to smoking, hypertension, diabetes and cardiovascular disease. But this is not a contest over which generation lived better. Each generation inherited a different health environment. For older adults, stopping smoking and controlling blood pressure and diabetes remain important. For younger adults, preventing these conditions from becoming established may offer an even greater opportunity.
So, are Gen Z kidneys better off?
It is too early to know. If Gen Z continues to smoke less and avoid harmful drinking patterns, that could prove beneficial over a lifetime. But the advantage could be diluted by years of inactivity, poor sleep, high-salt diets, obesity, hypertension and diabetes. The kidneys do not recognise generations. They respond to biology — blood pressure, blood sugar, vascular health, body weight, diet, physical activity and the cumulative effect of everyday choices. Perhaps Gen Z's real advantage is not that it has fewer risks. It is that it has more information about them. The challenge is turning that information into behaviour. Because the kidneys rarely complain when damage begins. By the time they do, the story may already have been unfolding for years.
